Reports to prevent future deaths - are Coroners wasting time?

Synopsis of talk:
Paragraph 7 of Schedule 5, Coroners and Justice Act 2009, provides coroners with the duty to make reports to a person, organisation, local authority or government department or agency where the coroner believes that action should be taken to prevent future deaths. These reports are described as Prevention of Future Deaths (PFDs) reports, and replace the Rule 43 reports of the previous legislation. PDFs are readily available on the website of the Chief Coroner, and provide a potential source of information about common, and uncommon, causes of preventable deaths. In practice, this potential may be only partially exploited because different coroners use PDFs differently, with very large differences in the numbers generated from different jurisdictions. Coroners are only required to identify potential hazardous situations, and are not expected to recommend solution. Individuals or organisations receiving PDFs are expected to reply to the PFDs, but there is no penalty if they don’t, and there is no compulsion to act on the implicit recommendations.

The talk will be designed to give an overview of the Post mortem CT service in Preston.

- To understand the differences between invasive post mortem versus CT post mortem services
- To understand the advantages and disadvantages of CT post mortem and when not to perform CT
- Be aware of differences of imaging and reporting techniques when looking at CT post mortem scans when compared to looking at CT scans in the living.

Pearls and Pitfalls from a Coroner’s perspective – can inquests lead to an improvement in care?

Synopsis of talk:
This talk will explore the legal purpose of an Inquest, and how the development of inquest proceedings have led to them becoming more adversarial than inquisitorial. It will also focus on the purpose of an Inquest away from the legal principles: to highlight opportunities to improve the provision of healthcare and to perfect imperfections.
