These reports are described as Prevention of Future Deaths (PFDs) reports, and replace the Rule 43 reports of the previous legislation. PDFs are readily available on the website of the Chief Coroner, and provide a potential source of information about common, and uncommon, causes of preventable deaths. In practice, this potential may be only partially exploited because different coroners use PDFs differently, with very large differences in the numbers generated from different jurisdictions. Coroners are only required to identify potential hazardous situations, and are not expected to recommend solution.
