Dr A Bom - "The development of Sugammadex"

[Despite 60 years of effort, the pharmaceutical industry has failed to develop a non-depolarising alternative for succinylcholine. However, in 1997 a new concept was discovered which might provide an even better option: the combination of rocuronium with an encapsulating agent, now known as sugammadex. This combination will allow the rapid induction of neuromuscular blockade, maintenance of blockade at any desired level and rapid reversal of any degree of neuromuscular blockade, including profound blockade. Sugammadex has the potential to change the way neuromuscular blocking agents are used in clinical practice. In the presentation Dr Bom will describe the discovery of the concept, the creation of sugammadex and its pre-clinical and clinical development.}

Presidential Address by

Dr R Postlethwaite
-
“The loneliness of the long distance medical manager”

[One recent reformulation of the principles of the Hippocratic Oath states that 'In the course of treatment, the physician is obligated to the patient and no one else.  He is not the agent of society, nor the interests of medical science, nor of his co-sufferers or future sufferers from the same disease.  The patient alone counts when he is under the physicians care'. I will argue that this is too simplistic a view and that this is particularly the case in a medical system which attempts to provide care based on the needs of the patient.  This could be conceptualised as the care of the next patient outside.  Doctors must engage in discussions about the allocation of resources and must engage in management.]

Dr N J Shaw - “The evidence base for treatment of chronic lung disease in neonates”

[The mainstay of treating established chronic lung disease in the neonate, is maintaining oxygenation, establishing adequate growth and minimising respiratory symptoms. Whilst in some areas there is little to guide clinicians in managing this disease, in others there is some evidence in the literature to inform treatment strategies.]

[The USA and many countries in Europe have primary care paediatricians.  In the UK we are seeing more children and young people presenting directly to Emergency Departments without seeing their GP.  Some of our health outcomes are not as good for children when compared to Europe.  Does this mean we should develop a system of primary care paediatrics?]

[Paediatric endocrine disorders, such as congenital adrenal hyperplasia, intimately tie the developing adrenal cortex to the process of sex differentiation in human females.  Our recent work studying early development has revealed a delicate balance to this critical aspect of female development under normal circumstances. The results carry implications for understanding normal human biology as well as syndromes of female virilisation and male under-virilisation.]

Dr F Karpe - "Adipose tissue: a useful organ or just blubber?"

[Adipose tissue is essential for whole body function and most processes within the tissue are highly regulated such as transport of storage of fatty acids, blood flow and secretions of hormones. However, the dynamic properties of the tissue are less evident in obesity and the organ is almost becoming a liability for the rest of the body with close associations to development of diabetes and cardiovascular risk factors. The mechanisms underlying this transition are still poorly understood.]

[Modern scientific studies of ancient Egyptian mummies provide evidence about lifestyle, diet, disease, medical treatments and religious beliefs and practices. This lecture will focus on the research carried out at the University of Manchester's unique specialist centre for biomedical Egyptology. It will include the latest studies undertaken on a renowned mummy in the Ulster Museum, Belfast, which will feature in the forthcoming BBC television documentary.]

Professor A M Heagerty -
"Fat cells, your arteries and your fate"

[There is increasing evidence that the fat cells that surround our arteries are highly active in altering the functional properties of the circulation. When there is a breakdown in the normal activity of adipose sites there is an increasing likelihood of the development of diabetes and hypertension. As our understanding of the metabolism of fat cells grows it is clear that the balance between the generation of inflammatory cytokines and protective hormones is finely tuned. However, in obesity there is clear evidence that fat cells enlarge and become hypoxic and when this occurs there is a breakdown in this equilibrium and the inflammation that follows is sufficient to cause a loss of circulatory homeostasis and an interference with glucose metabolism and the generation of accelerated atherosclerosis with the attended risks of premature death.]
